Telehealth Medical Consent

1. Nature of Telehealth
Telehealth involves delivering healthcare services using electronic communication technologies. This may include video consultations, messaging, or digital questionnaires.
You are engaging with an independent, licensed healthcare Provider, not with Cedar Clinic directly. The Provider will review the health information you provide and, if appropriate, prescribe medication. This is not an ongoing primary care relationship. You should continue to see your regular healthcare provider for comprehensive medical care.
2. Risks of Telehealth
Potential risks include:
- Technical failures, including internet disruptions or software errors that could interrupt or delay care.
- Incomplete information due to electronic communication, which may limit the Provider’s ability to fully assess your condition.
- Delays in evaluation or treatment.
- Security risks, including the possibility of unauthorized access to electronic transmissions (though we take significant steps to prevent this).
- The Provider may determine that telehealth is not appropriate for your situation and may recommend an in-person evaluation.
3. Benefits of Telehealth
Benefits may include:
- Improved access to healthcare providers.
- Convenience and flexibility — faster access to GLP-1 weight-loss consultations and prescriptions.
- Reduced travel time and cost compared to traditional office visits.
4. Patient Responsibilities
You agree to:
- Provide accurate and complete medical information, including all current medications, supplements, and health conditions.
- Inform providers of any changes in your condition.
- Seek emergency care when appropriate. Telehealth is not appropriate for emergencies — call 911 or go to the nearest emergency room immediately if you are experiencing a medical emergency.
5. Consent to Treatment
By using the Platform, you consent to receiving telehealth services from licensed healthcare providers. You may withdraw consent at any time by discontinuing use of the Platform.
You have the right to withhold or withdraw consent to telehealth services at any time without affecting your right to future care, and to request an in-person evaluation if clinically appropriate.
6. GLP-1 Medication Consent
If you are prescribed a GLP-1 receptor agonist (such as semaglutide or tirzepatide), you acknowledge:
- Common side effects may include nausea, vomiting, diarrhea, constipation, stomach pain, and decreased appetite.
- Serious risks may include pancreatitis, gallbladder disease, kidney problems, and — in animal studies — thyroid tumors (clinical significance in humans is unknown).
- GLP-1 medications are contraindicated in individuals with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
- You must disclose all current medications, supplements, and health conditions to your Provider.
- You are not currently pregnant, planning to become pregnant, or breastfeeding. GLP-1 medications should be discontinued at least 2 months before attempting conception.
- Results vary. Weight loss is not guaranteed and depends on adherence, lifestyle, and individual factors.
